Times, code/examples/tables in non-proportional Courier, titles, nice
tables and all kinds of visual enhancements to improve
readability. *NO* changes have been made to the contents of the
olriginal document.  This document has been uploaded with the
permission of the author Wouter van Oortmessen (he said he liked it
